Job summary

Asian Health Services in California is seeking an Activity Coordinator to lead a comprehensive therapeutic recreation program for PACE participants. You will assess needs, plan care, and coordinate group and individual activities within the Interdisciplinary Team.

The role requires fluency in Chinese (Cantonese or Mandarin), at least two years in a related setting with a relevant bachelor’s degree, and the ability to conduct home visits. Epic is used for documentation and workflow.

Qualifications

• Fluency in Chinese (Cantonese or Mandarin) required.
• Two years of experience in a social recreational or educational program within the past five years, including one year full-time in a health care, mental health, or handicapped setting.
• Bachelor’s degree in related field from an accredited university or college.
• Ability to make home visits as assigned.
• Knowledge of social activities, including programs for older adults.

Responsibilities

• Coordinates and implements day center social programming and activities with culturally and linguistically appropriate communication.
• Conducts assessments and participates in Participant care planning; sets individualized goals for social/recreational programs.
• Oversees day room activities with a focus on safety and risk mitigation.
• Documents progress in the Participant’s electronic health record.
• Develops and oversees the daily Recreation Program, including group/individual activities and community outings.
