Job summary

Trilogy in Chicago is seeking a Team Leader for the Assertive Community Treatment (ACT) team. You will supervise staff, oversee service quality, hire and mentor clinicians, and partner with managers and external partners to improve outcomes for clients with severe mental illness.

The role emphasizes client-centered, strengths-based, trauma-informed care with harm reduction, and involves coordinating care across disciplines and managing team performance and engagement.

Qualifications

• Master’s degree in a related field is required.
• Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) is required.
• Minimum two years experience working with people with mental health and/or substance abuse diagnoses.
• Valid Illinois driver's license with daily access to a vehicle and proper insurance.
• Supervisory or leadership experience is highly preferred.

Responsibilities

• Complete weekly supervision with employees to provide feedback and follow-up on duties.
• Assess team morale and maintain positive team culture.
• Model a client-centered, recovery-oriented approach using trauma-informed care and harm reduction.
• Coordinate care with interdisciplinary staff and family to ensure continuity.
• Refer clients to appropriate services and monitor care levels.
• Track team capacity to maintain staff-to-client ratios.
• Ensure documentation (IM-CANs, notes) is complete and timely.
• Assist with hiring, onboarding, and ongoing staff training.
• Provide community-based services and weekend support as needed.
• Monitor trainings and ensure staff stay current with required certifications.
