Job summary

TRILOGY BEHAVIORAL HEALTHCARE is seeking a Team Leader to serve as the senior clinician on the Community Support Team in Chicago. You will supervise staff, coordinate services, and ensure quality care for clients with severe mental illness and co-occurring substance use disorders, using a harm-reduction approach.

You will hire, train, coach, and mentor clinicians, interfacing with managers and external partners to improve program effectiveness and outcomes.

Qualifications

• Master’s degree in social work, psychology, counseling, or closely related field.
• Minimum two years of experience with mental health and/or substance use diagnoses.
• Supervisory or leadership experience preferred.

Responsibilities

• Provide weekly supervision and feedback to team members.
• Monitor team morale and maintain positive culture.
• Model client-centered, recovery-oriented care using evidenced based practices.
• Coordinate care with interdisciplinary staff and families for continuity.
• Refer clients to higher levels of care when needed; manage referrals.
• Track team capacity to maintain appropriate staff-to-client ratios.
• Assess clients’ biopsychosocial needs through home, community, and office visits.
• Support crisis interventions and assist with independent living skills and entitlements.
• Schedule daily client visits and run regular team meetings.
• Assist with hiring, onboarding, and ongoing staff training.
• Monitor documentation and annual trainings compliance.
