Catholic Charities of the Diocese of Rochester seeks a dedicated professional to oversee the Self-Directed Services program. You will act as employer of record, conduct background checks, manage payroll/benefits, and ensure services align with approved plans and budgets.

Qualifications

• Associates degree or 60 hours of college study in human services and 3+ years' experience with individuals with intellectual/developmental disabilities; supervisory experience preferred.
• Bachelor's degree in human services and 2+ years' experience with individuals with intellectual/developmental disabilities; supervisor or trainer experience acceptable.
• Combination of education and experience may be substituted at the Executive Director's discretion.

Responsibilities

• Act as direct contact/liaison for participants, the local DDRO, Care Managers, and Brokers considering CCFCS for Self-Directed services.
• Provide Fiscal Intermediary Services to participants and families with approved self-directed budgets.
• Provide training and support to self-directed participants and families, including hiring and supervising staff, regulatory guidance with self-directed waiver services and other trainings.
• Ensure Direct Support Professional Core Competencies are addressed and evaluated.
• Provide guidance to participants and families and supervise self-direct staff when job performance or conflicts require disciplinary action.
• Gather timesheets, mileage invoices, and reimbursement forms and ensure payments align with the approved plan/budget.
• Review Life Plans, Staff Action Plans, and monthly summaries for accuracy and compliance.
• Maintain documentation of overall program operations for each Self-Directed participant.
• Attend COS, Life Plans, and Launch meetings to review supports and services authorized by the Self-Directed Plan/Budget.
• Verify and process invoices/service records for contracted/vendor services and maintain required documentation.
