Job Overview

Self Directed Services Staff – Watts Catholic Charities of the Diocese of Rochester Location: Client Dependent / Community Based Schedule: Varied, dependent on client need Salary: $15.50/hour for administrative work; rate for client work set by client/family

Essential Duties & Responsibilities
  • Assist individuals with specific outcomes as described in each individualized Self-Directed Plan. Duties may include personal care, community integration, money management, advocacy, employment needs, and respite.
  • Seek opportunities to meet individuals’ psychosocial needs through planned and spontaneous actions.
  • Provide reliable, safe, high‑quality services as directed, in a thorough and caring manner.
  • Implement a person‑centered approach to services.
  • Follow universal precautions, proper infection control, sanitation, and safety standards of practice in all work and activities.
  • Complete paper timesheet/invoice and web‑based time‑keeping promptly in accordance with payroll processing.
  • Communicate with supervisor and Circle of Support about an individual’s habilitation plans that may need revisions or updates.
  • Communicate effectively with individuals with intellectual/developmental disabilities, Circle of Support, broker, fiscal intermediary coordinator, service coordinator, supervisor, interagency professionals, community professionals, and families.
  • Work consistently with all team members, led by the individual and family preference to meet the needs and desires of the individual served.
  • Respect the individual’s dignity, privacy, property, religion, and culture.
  • Document and maintain accurate records of contacts with individuals.
  • Provide appropriate interventions to all crisis situations. Notify immediately of any allegation of abuse or neglect.
  • Provide transportation as needed to individuals and follow safety procedures regarding transportation.
  • Maintain flexibility in schedule based on needs of individuals assigned.
  • Be dependable and arrive on time, ready for responsibilities.
  • Participate in relevant agency meetings and/or trainings.
  • Pursue development of professional competencies related to job role performance through reading, supervision, in‑service training, and attendance at conferences and workshops.
  • Be aware of and actively support the agency’s mission, vision, values, and strategic plan, including core competency initiatives (collaborative, community‑focused, innovative, entrepreneurial, culturally competent, and diverse).
  • Perform other duties as assigned by individual being served or CCFCS SDS Supervisor.
Qualifications & Requirements
  • High School Diploma or equivalent preferred. Relevant combination of education and experience will be considered.
  • Clean and valid NY State Driver License and access to a reliable and insured vehicle if driving is required. Anyone with driving responsibilities must be at least 21 years old.
  • At least 1 year of practical experience working with individuals with intellectual/developmental disabilities preferred.
  • Ability to work a combination of day, evening, overnight, and weekend hours as needed.
  • Ability to effectively communicate with, teach, monitor, and assist individuals with intellectual/developmental disabilities.
  • Ability to work both independently and cooperatively as a team member.
  • Ability to ascend and descend stairs.
  • Ability to provide physical interventions such as lifting, grasping, holding as necessary.
  • Commitment to agency mission statement.

  • Foster agency, department, and program‑wide cooperation and teamwork through positive and constructive communication techniques.
  • Maintain absolute confidentiality of all records reviewed.
