Job summary

Livingston County Community Mental Health Authority seeks a Self-Determination Coordinator I/II to coordinate across the full network supporting self-determined services. You will educate consumers and families on options, manage self-determination agreements, and monitor budgets with MDHHS policy compliance.

You will lead trainings, develop policies, and drive quality improvement, using data to inform leadership. A strong collaboration with external partners and internal teams is essential.

Qualifications

• Bachelor's degree in Social Work, Psychology or related field.
• Master's degree in Social Work, Psychology or related field.
• Proficiency with Microsoft Office and electronic medical records systems.

Responsibilities

• Coordinate across the network that supports self-determined arrangements and educate consumers and families on self-determination options.
• Issue and track self-determination agreements; formulate and monitor budgets; analyze data and trends.
• Lead training and orientation for staff, EORs, families, and individuals; develop policies and quality indicators.
