Qualifications

Responsibilities

• Coordinate the Self Determination program with consumers and families.
• Develop the IPP in conjunction with the Person-Centered Plan.
• Gathers data, conducts assessments, and maintains case records.
• Advocates to secure needed services and ensure compliance with regulations.
