Job summary
At Peace Health Care Agency in Pennsylvania seeks an experienced Direct Support Professional (DSP) to provide person-centered care to individuals with IDD in home and community settings. You will support daily living needs, behavioral support, personal care, and community participation.

Qualifications
- Previous Direct Support Professional experience required.
- Experience supporting individuals with IDD and/or Autism.
- Experience with behavioral needs and BSPs preferred.
- Knowledge of person-centered care and individual rights.
- Strong communication and problem-solving skills.
Responsibilities
- Provide direct support with ADLs, personal care, mobility, and daily routines.
- Support individuals with IDD, Autism Spectrum Disorder, and behavioral needs.
- Implement Individualized Support Plans and Behavioral Support Plans (BSPs).
- Promote independence, dignity, choice, and community involvement.
- Provide companionship and support during community activities and appointments.
- Assist with transportation as needed.
- Monitor and document progress, services, and daily activities accurately.
- Maintain a safe, respectful, and person-centered environment.
- Protect individual confidentiality and follow HIPAA and agency policies.
- Communicate effectively with individuals, families, and the care team.
