Job summary

Maryland Physicians Care seeks a seasoned Social Work leader to co‑manage the Social Determinants of Health (SDoH) strategy. You will guide regional teams of social workers and community health workers to address complex needs while aligning with MPC leadership across departments.

The role emphasizes data‑driven performance insights, scalable workflows, and partnerships that improve outcomes and reduce costs for MPC members, including Medicaid populations.

Qualifications

• Understanding of social determinants of health and health disparities.
• Ability to coordinate care across social work, community health workers, and external partners.
• Experience with Medicaid/Medicare managed care environments.

Responsibilities

• Lead regional SDoH teams to screen, assess and connect members with social care needs.
• Advance integrated social care models across MPC departments and partners.
• Provide supervision and coaching to social workers and community health workers.
• Oversee care planning, crisis planning, and barrier mapping for high‑risk cases.
• Collaborate with external vendors and internal teams to scale SDoH initiatives.
