Job summary

Jobtailor is seeking a social work leader to co-manage Social Determinants of Health (SDoH) initiatives in Maryland. The role focuses on complex cases and aligns with MPC’s objectives to improve member outcomes through collaborative care.

You will guide regional teams of social workers and community health workers, drive data-driven performance metrics, and scale scalable workflows while ensuring ethical practice and compliance with reporting requirements.

Qualifications

• Demonstrates expertise in Social Determinants of Health (SDoH) programs and data-driven strategies.
• Experience coordinating care across multidisciplinary teams in health programs.
• Ability to lead and scale social care initiatives within Medicaid/Medicare contexts.

Responsibilities

• Co-manage SDoH programs and align with MPC strategy and vision.
• Lead regional multidisciplinary teams of social workers and community health workers.
• Ensure coordinated, high-quality, and cost-effective interventions.
• Advance scalable workflows and collect performance metrics.
• Use data-driven insights to target services to high-need members.
• Drive integrated social care models through cross-department collaboration.
• Manage social work ethics and mandated reporting.
