Job summary

TalentLNX LLC in Baltimore, MD is seeking a Social Determinants of Health (SDOH) Navigator to connect high-risk members with community resources and social services that support their health and well-being. This full-time, on-site role reports to care management and works with a cross-functional team.

You will screen for social needs, assist with Medicaid, SNAP, LIHEAP, and local benefits, and follow up to ensure resources are connected.

Qualifications

• Associate degree in Human Services, Social Work, or equivalent experience.
• 1+ year in community health, social services, or patient navigation.
• Knowledge of community resources and social service systems.
• Familiarity with SDOH frameworks and screening tools.
• Empathetic communication with diverse community members.

Responsibilities

• Screen high-risk members for social needs using validated tools.
• Connect members with housing, food, transportation, utilities, and financial support.
• Assist with Medicaid, SNAP, LIHEAP, and local benefits applications.
• Coordinate with care managers and social workers on complex cases.
• Document interactions and track referral outcomes.
