Job summary

AbsoluteCare Medical Center & Pharmacy is seeking an SDOH Coordinator to support members by linking them to community resources and assisting with applications and follow-up.

The role requires knowledge of local resources, eligibility processes for benefits, and the ability to coordinate with healthcare teams to ensure access to housing, SNAP, transportation, food, and other essential services. CPR/BLS certification is required.

Qualifications

• Bachelor's degree or 3 years' experience serving complex populations.
• 2 years' experience serving complex populations (Medicaid, Medicare, uninsured).
• Extensive knowledge of local resources and entitlement/application processes.
• Strong teamwork to work within an integrated care team.
• Strong written and oral communication skills.
• Strong interpersonal skills to build relationships with staff, members, and stakeholders.
• Ability to work independently to complete tasks.
• Proficiency with Outlook, Word, Excel, PowerPoint.

Responsibilities

• Receive referrals from the Integrated Care Team to address members' SDOH needs.
• Assist members with referrals and coordinating services with community resources.
• Update the local Center's Community Resource Guide when needed.
• Collaborate with ICT related to services not covered by the health plan.
• Exhaust all community and health plan options before requesting funds for SDOH items.
• Work with the local Resource Specialist on member fund requests for SDOH expenses.
• Coordinate with Behavioral Health for referrals and services.
• Participate in integrated care team huddles and population health meetings.
• Follow primary care provider action plan to meet member needs.
• Coordinate with licensed behavioral health professionals for clinical referrals.
• Engage members in-center and in the community; document interactions in the EHR.
• Prioritize tasks to meet response time performance metrics.
