Job summary

AbsoluteCare Medical Center & Pharmacy is seeking an SDOH Coordinator to assist primary care teams with complex social needs and connect members to community resources. The role involves referrals, benefits applications, and follow-up to ensure access to resources.

Key duties include coordinating housing, benefits, food, transportation, and other support services, plus documenting activities and collaborating with ICT and Behavioral Health teams as needed.

Qualifications

• Bachelor’s degree or related field with 3 years’ experience serving complex populations.
• Extensive knowledge of local community resources and entitlement processes.
• Strong teamwork and communication skills.

Responsibilities

• Receive referrals and address members’ SDOH needs.
• Assist with referrals and coordinating services with community resources.
• Update the Community Resource Guide as needed.
• Collaborate with ICT and other teams to cover services.
• Work with the Behavioral Health team for mental health referrals.
• Document interactions in the electronic health record.
• Prioritize tasks to meet response time metrics.
• Engage members in the center and in the community.
