Job summary

AbsoluteCare Medical Center & Pharmacy in Philadelphia, PA is seeking an SDOH Coordinator to assist primary care teams by connecting members with community resources and benefits, and by guiding them through eligibility and application processes.

You will coordinate referrals, maintain knowledge of housing, SNAP, transportation, LIHEAP, and other supports, and document interactions in the electronic health record to ensure timely access.

Qualifications

• Bachelor's degree or equivalent experience in public health or related field.
• 3+ years serving complex populations (Medicaid/Medicare/uninsured) preferred.
• Strong communication and teamwork required.

Responsibilities

• Assist ICT referrals to address SDOH needs identified during assessment.
• Coordinate services with community resources including housing, SNAP, transportation, and utility support.
• Update the local Community Resource Guide as needed.
• Collaborate with ICT and Behavioral Health teams to facilitate referrals.
• Document all member interactions in the EHR with timeliness.
• Prioritize tasks to meet response time metrics.
