Job summary

BCforward is seeking a Community Engagement Navigator for a 4-month contract in Clyde, NC. Start ASAP. The role focuses on removing SDOH barriers and coordinating with referral sources to connect members with appropriate providers or programs.

Responsibilities include establishing member goals, aligning plans with case management, and maintaining strong communication. Travel to worksites may be required; BA/BS in a related field and relevant experience are expected.

Qualifications

• BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field; 2 years experience in marketing, job development, placement or social work.
• Knowledge of Voc Rehab preferred.
• Willingness to travel to worksites as necessary.

Responsibilities

• Engage with members to remove barriers to Social Determinants of Health (SDOH).
• Act as liaison between the company and referral sources.
• Help members establish goals and next steps; assist in selecting providers or programs.
• Interface with HCM to align individualized plans with case management; multitask in a fast-paced environment.
