Essence Community Care in Chicago seeks a Social Care Specialist to support individuals with healthcare and social service needs. This role manages a caseload, performs SDoH assessments, coordinates benefits navigation, and advocates for participants through community resources.

The position is hybrid (Chicago-based) with in-office work, remote work, and in-community events. Ideal for someone organized, compassionate, and experienced in Medicaid/Medicare and public benefits.

Formación

• Min 3 years in social services, care coordination, or related fields.
• Experience with SDoH and public benefits.
• Knowledge of Medicaid, Medicare, SNAP, housing/resources.

Responsabilidades

• Engage participants with compassion and trauma-informed care.
• Conduct comprehensive SDoH assessments.
• Manage caseloads with moderate to complex needs.
• Develop individualized care plans.
• Assist with accessing healthcare coverage and benefits.
• Provide renewal support and eligibility guidance.
• Coordinate referrals and warm handoffs.
• Monitor progress and follow-ups.
• Maintain confidentiality and HIPAA compliance.
• Document interactions in internal systems.
• Support team with guidance and training.
• Stay updated on local resources and programs.
