AWARE is seeking an SDMI Adult Case Manager to support adults, children, and families in our Montana community. You will work within the SDMI Waiver program, assess needs, develop and monitor Individual Treatment Plans, and help clients access services.

Qualifications

• BS/BA degree in a social work-related field and minimum of one (1) year of direct work experience in adult mental health case management.
• One (1) year experience in the human service field.

Responsibilities

• Caseload and/or billing requirements must be met on a monthly, quarterly, and yearly basis.
• Maintains a caseload in collaboration with the Clinical Administrator for SDMI programs in assigned areas.
• Assesses and evaluates adults to determine appropriateness for case management and SDMI Waiver services.
• Provides coordination and development of an Individual Treatment Plan (ITP) and monitors it as required.
• Keeps records and enters data in AWARE’s Electronic Healthcare Record System.
