Sage Connections, LLC is hiring full-time W-2 Licensed Practical Nurses (LPNs) to provide K–12 school-based services for the 2025–2026 school year. Onsite in Pittsburgh, PA, with a 37.5–40 hour workweek, Monday–Friday.

Caseload largely 1:1, supporting students and coordinating care with families and school staff. Requirements include an active PA LPN license, PDE Educational Specialist Certification in the relevant area (PK-12 preferred), CPR/First Aid, and an Associate’s degree in Nursing.

Qualifications

• Current and Unrestricted Pennsylvania Nursing License (LPN)
• PDE Educational Specialist Certification in School Nurse PK-12 or equivalent preferred
• Associate's degree in Nursing preferred, Bachelor’s optional

Responsabilités

• Manage student health records and medication administration
• Support chronic health condition management (e.g., asthma, diabetes, allergies)
• Coordinate with parents, school staff, and outside providers as needed
• Maintain documentation and comply with health-related regulations
• Participate in IEP or 504 Plan meetings when relevant

• Caseload: Multiple positions available, mostly 1:1 support positions
• Schedule: Monday – Friday
