Job summary

Sage Connections, LLC is seeking a dedicated Registered Nurse (RN) to provide 1:1 health support for a high school senior during the 2026-2027 school year. This full-time W-2 position is onsite in Middletown, with a Monday–Friday schedule and 30 hours per week.

Qualifications

• Pediatric nursing experience in a school or clinical setting.
• Active RN license and CPR/First Aid certification.
• Strong organizational and communication skills for student-centered care.

Responsibilities

• Provide 1:1 RN nursing support to a high school senior with significant needs.
• Manage diabetes care and other health needs during the school day.
• Support activities of daily living and mobility as needed.
• Assist with wheelchair transfers and safe handling.
• Support academic activities and vocational exploration.
• Coordinate with parents, school staff, and healthcare providers.
• Maintain health documentation and comply with regulations.
• Participate in IEP/504 Plan meetings when relevant.

• Caseload:1:1 support for male, high school senior.
• Schedule:Monday - Friday
• Hours:30 hours/week
• Dates:10/4/26 - 6/17/26
• Location:Onsite in Middleton

Responsibilities

• Provide 1:1 nursing support to a high school senior in a significant needs classroom
• Manage diabetes care and other health needs throughout the school day
• Support ADLs including feeding, toileting, transfers, and personal care
• Assist with wheelchair mobility and 2-person lifting as needed
• Provide academic and classroom support, as well as community vocational exploration
• Coordinate with parents, school staff, and healthcare providers as needed
• Maintain documentation and comply with school health and safety regulations
• Participate in IEP or 504 Plan meetings when relevant

Qualifications

• Current and Unrestricted Rhode Island RN License
• 1+ years of pediatric nursing experience
• Experience supporting students with complex medical and developmental needs preferred
• Experience with diabetes, Down Syndrome, Cerebral Palsy, and wheelchair support a plus
• CPR and First Aid certification required
• Strong organizational, communication, and decision-making skills
• Compassionate, patient, and student-centered
