Job summary

Primary Health Solutions is seeking a Youth Community Psychiatric Support Specialist to provide community-based mental health services for youth and families. You will develop mutual treatment plans, deliver interventions, and collaborate with a larger care team to ensure client-centered care in a dynamic, field-based setting.

Qualifications

• Associate's or Bachelor's Degree in the human services field.
• Experience providing services to families and children preferred.
• Access to an operational vehicle and a valid driver's license required.
• Auto insurance with 100,000/300,000 liability coverage maintained.

Responsibilities

• Establish rapport with youth and families and assess treatment needs; identify strengths to aid goals.
• Meet with youth and families face-to-face; provide mental health interventions at least once per month.
• Coordinate ongoing assessment of needs and monitor self-management of symptoms.
• Provide crisis intervention when needed and assist in linking to community resources.
• Maintain clinical records with timely documentation and collaborate with other providers to ensure client-centered care.
