Job summary

Centerstone is seeking a licensed clinician to provide therapeutic interventions and oversee growth plans for clients with behavioral health needs. You will conduct intakes, develop treatment plans with the care team, and monitor progress with ongoing documentation to support recovery and resilience.

The role includes discharge planning, care coordination with external resources, and supervision of practicum students.

Qualifications

• Master's degree required in social work, psychology, or related field.
• 2 years of post-licensure experience preferred.
• Licensure required: LISW, LCSW, LMHC, LPC, or LPCC.
• Strong communication and collaborative skills.

Responsibilities

• Facilitates therapy sessions for a client or group of clients.
• Develops and updates treatment plans with the care team.
• Conducts intake for new services and ensures consent.
• Monitors client progress and documents assessments.
• Assists with discharge planning and follow-up.
• Supervises practicum and internship students and staff.
