Job summary

Center for Human Development (CHD) seeks a part-time School-Based Community Health Worker for our Community School Based Clinic in Franklin County, MA. The role collaborates with Clinicians and school staff to help students and families access on-site tele behavioral health services and community resources.

You will document encounters, collect data, and assist with referrals and required authorizations while maintaining timely monthly reports.

Qualifications

High School Diploma or Equivalent required.
Strong computer skills- Electronic medical record (EMR) databases.
Knowledge of area community agencies and resources.
Skill in organizing resources and establishing priorities.
Creative and analytical thinking.
Strong interpersonal and communication skills.

Responsibilities

Provide home, school and community-based care coordination to youth and their families.
Assist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed).
Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals.
Provide ongoing follow-up, motivational interviewing and goal setting with students/families.
