Qualifications

• Master's degree in social work required.
• Licensed Social Worker NYS LMSW or LCSW.
• BLS Certification required at time of hire and maintained.

Responsibilities

• Provide care based on physical, psychosocial, educational, safety and age criteria for patients.
• Perform thorough intake assessments and ongoing evaluations.
• Develop accurate assessments and diagnoses and implement treatment plans.
• Engage family or significant others to support comprehensive care.
• Seek consultations with treatment team psychiatrist or NP as needed.
• Document in compliance with clinic, hospital and regulatory standards.
