Job summary

Responsibilities

• Conduct community outreach and SDOH screening.
• Screen for benefit and program eligibility.
• Coordinate on-site legal clinic scheduling and referrals.
• Assist clients with benefit applications and resource navigation.
• Document in EPIC and other platforms; participate in required meetings.
