Job summary

Family Health Centers at NYU Langone is seeking a Community Health Worker (Spanish Speaking Required) to join a multidisciplinary primary care team. The role emphasizes engaging patients in services, educating self-care, and facilitating access to care to improve health and quality of life.

The position requires a High School Diploma/GED, at least 1 year in a health-related field, and bilingual abilities (English/Spanish or other languages as needed).

Qualifications

• High School Diploma or GED required.
• At least 1 year health service related experience.
• Bilingual English and Spanish or other targeted language as per patient population.
• Computer literacy including Microsoft Office; able to use telephone, fax, and copier.

Responsibilities

• Assist patients in obtaining cancer screenings and follow-up care.
• Assess patients for social drivers of health.
• Connect patients to medical, behavioral health, chemical dependency, and community services.
• Participate in required trainings and team meetings.
• Advocate for patients to overcome barriers to care including language, transportation, and insurance issues.
• Provide education and coaching to parents to improve health outcomes.
• Document all patient interactions in Compass Rose EPIC per standards.
• Maintain caseload and assist with benefit applications and connections to services.
