Qualifications

• Active Speech Language Pathologist licensure in the state of assignment.
• CCC-SLP certification preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice.
• CPR current if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current health certificate per contract or state regulation
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

Responsibilities

• Perform student evaluations and document therapy services per plan of care.
• Provide therapy services in the school setting according to physician orders.
• Maintain accurate documentation and records for each student.
