Job summary

Western New York Integrated Care Collaborative, Inc. seeks a School Based Physical Therapist (PT) to assess, plan, and implement rehabilitative programs within educational settings.

The role focuses on improving mobility, reducing pain, and enhancing functional performance for students. The candidate should hold a Master's degree in Physical Therapy with NYS licensure and strong communication, documentation, and collaboration with school personnel.

Qualifications

• Knowledge of English language including meaning and spelling of words, rules of composition and grammar to document services.
• Skills in providing direct service; evaluation, consultation and monitoring of programs using logic and reasoning.
• Written and verbal communication to supervisors, co-workers, families, school personnel, and subordinates.
• Time management to work independently and maintain documentation per regulations and OTAS standards.
• Skills in staff training and development.
• Ability to direct others as necessary.

Responsibilities

• Knowledge of computer systems and ability to work with word, google docs, and data analysis programs.
• Document student’s condition, treatment program, and necessary functional needs to achieve educational goals.
• Plan, prepare, and carry out an individually designed program of treatment.
• Identify and document goals, anticipated progress and program adjustment, if necessary.
• Prepare and maintain student progress notes in a timely manner.
• Discuss the students progress with appropriate school personnel and with the family, including home treatment procedures.
• Schedule students for physical therapy services.
• Recommend appropriate adaptive equipment or supplies needed by the student.
• Maintain CEU’s per NYSED for Physical Therapist License.
• Maintain a safe work environment and report unsafe conditions.
• Report all on-the-job injuries to the OTAS office as soon as they occur.
• Placement to include therapy room, classroom, teletherapy, in home, and neutral site as indicated by district.
