Northshore Home Health Care is seeking a School Physical Therapist to provide therapy services to students within a school setting. You will assess movement dysfunction, develop treatment plans, and educate students to improve mobility and reduce pain.

Join a collaborative team, communicate with families and school personnel, and deliver evidence-based therapies while ensuring accurate documentation and compliance with NY licensure requirements.

Qualifications

• Must hold an active NY PT license.
• Graduate from an accredited DPT program.
• Pass the NPTE.
• Strong communication skills.
• Previous school-based experience is a plus.
• Completion of background checks is required.

Responsibilities

• Conduct assessments, create personalized treatment plans, deliver direct and consultative therapy.
• Report findings and progress to students, families and school staff.
• Support student functional improvements within a collaborative team.
