Job summary

Bond Health in New York seeks a Physical Therapist to evaluate, diagnose and provide therapeutic intervention to students under IFSP/IEP guidelines across programs in five boroughs.

Responsibilities include annual PT evaluations, treatment planning, progress notes, collaboration with parents and an interdisciplinary team, supervision of students and assistants, and ensuring services meet educational goals.

Qualifications

• Must have Bachelor's of Science in Physical Therapy.
• Must hold NYS Physical Therapy license.
• Strong communication and organizational abilities.

Responsibilities

• Provide PT evaluation of students annually.
• Develop treatment plans and goals based on assessment.
• Maintain attendance records and ensure services align with educational goals.
• Integrate therapy into programs and collaborate with parents and staff.
• Supervise Physical Therapy students and assistants; complete timely documentation.
