Job summary

Responsibilities

• Meet with patients from diverse backgrounds and discuss health concerns and therapy needs.
• Conduct evaluations to determine a starting point for each patient.

• Work closely with care team to provide follow-up care.
• Maintain accurate records of patient therapy and progress.
