You will collaborate with other health team members to arrange follow-up care and educate patients on lifestyle changes, while maintaining a positive, compassionate presence. Responsabilités

• Meet with patients from diverse backgrounds and discuss health concerns and therapy needs.
• Conduct evaluations to determine a starting point for each patient.
• Create a therapy plan specific to each patient to meet mobility goals.
• Work closely with care team to provide follow-up care.
• Maintain excellent records of patient therapy and progress.
