Job summary

Camden Family Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join its School-Based Health Center team within a Federally Qualified Health Center (FQHC). The role serves both school-based patients and community members, delivering comprehensive primary care with emphasis on preventive and acute care in a multi-site setting.

The position offers 5–8 hour days aligned with school hours, with no nights or weekends, and opportunities for loan repayment through

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and active licensure in the state of West Virginia
• DEA registration (if applicable)
• Experience in pediatrics, family medicine, or community health preferred
• Strong clinical assessment and decision-making skills
• Ability to work independently in both school-based and outpatient/community settings

Responsibilities

• Provide comprehensive primary care services
• Conduct physical exams, acute care visits, and well-child checks
• Diagnose and manage acute and chronic conditions within scope of practice
• Provide immunizations and preventive care services per guidelines
• Serve both school-based patients and community-based patients through designated clinic access points
• Document all patient encounters accurately and timely in the EHR
• Coordinate care with families, school staff, and interdisciplinary care teams
• Support care coordination, referrals, and continuity of care across settings
• Participate in health education and community wellness initiatives
• Ensure compliance with all FQHC, state, and federal clinical standards
