Job summary

The Neighborhood Center, Inc. seeks a School Based Case Manager to serve Schoharie County schools, assisting students with psychiatric rehabilitation access and ongoing support to reduce crisis involvement.

You will advocate for clients, link to services, monitor progress, and collaborate with school personnel to enhance outcomes. Responsibilities include coordinating with county clinics, documenting interactions, and providing educational presentations while maintaining professional standards

Qualifications

• Bachelor's degree in social work, psychology, or related field.
• Minimum of two years in a human services agency with one year in a mental health setting.
• Valid New York State driver's license and access to reliable transportation.
• Knowledge of mental health and substance use issues affecting children and adults.
• Ability to navigate system-level resources and service access.

Responsibilities

• Advocate, monitor, and serve as primary resource for assigned clients.
• Implements psychiatric rehabilitation principles and empowers clients.
• Conducts individual needs assessments.
• Encourages participation in appropriate programs and provides assistance as needed.
• Conducts crisis reporting and collaborates with Crisis Counselors and school staff.
• Builds collaborative relationships with community providers and coordinates services.
• Document interactions in web-based software and provide trainings as assigned.
• Attend case conferences and participate in supervision regularly.
• Maintain confidentiality and adhere to agency policies.

Education

Bachelor's Degree in Social Work, Psychology, or other closely related human service field
