Job summary

The Neighborhood Center, Inc. seeks a School Based Case Manager to serve Schoharie County schools, assisting students with psychiatric rehabilitation access and ongoing support to reduce crisis involvement.

You will advocate for clients, link to services, monitor progress, and collaborate with school personnel to enhance outcomes. Responsibilities include coordinating with county clinics, documenting interactions, and providing educational presentations while maintaining professional standards

Qualifications

• Bachelor's degree in social work, psychology, or related field.
• Minimum of two years in a human services agency with one year in a mental health setting.
• Valid New York State driver's license and access to reliable transportation.
• Knowledge of mental health and substance use issues affecting children and adults.
• Ability to navigate system-level resources and service access.

Responsibilities

Job description

JOB DESCRIPTION: The School Based Case Manager (SBCM) is responsible for assisting individuals with mental illness, within school districts in Schoharie County, who are in need of psychiatric rehabilitation to access acute care resources. The role of the SBCM is to maintain and enhance the functioning level of this population to establish healthy, constructive living patterns and reduce the rates of recidivism into acute services. The SBCM is expected to work, with a high degree of independence, to address the needs of individuals with mental illness by providing advocacy, monitoring, linking, and support services.

Specific responsibilities include

• Serves as an advocate, facilitator, monitor, supporter and primary resource for assigned clients.
• Implements the principles of psychiatric rehabilitation by encouraging the empowerment of each assigned client.
• Conducts individual needs assessments.
• Encourages client participation in appropriate programming services and provides assistance as needed.
• Assists clients in exercising better judgment skills through supportive monitoring.
• Supports and encourages clients as they attempt to adjust to new services.
• Reports crisis situations and collaborates with Crisis Counselors and school personnel when set-backs indicate the need for assessment and/or stabilization.
• Conducts outreach visits to high-risk individuals.
• Conducts face-to-face visits with students being discharged from Emergency Departments and inpatient psychiatric units and provides intensive post discharge follow-up contact.
• Good working knowledge of resources within the community.
• Collaborates with service providers to expedite access to services for clients.
• Builds and fosters the development of collaborative relationships with community service providers
• Makes recommendations and assists in facilitating the appropriate level of treatment, services and supports.
• Links clients by arranging appointments with service providers and facilitating the receipt of such services, as well as arranging for transportation when appropriate.
• Document all interactions in an accurate and timely manner in web-based software.
• Provides educational/ marketing /training presentations to the schools and the community, as assigned.
• Coordinate responses to county requests for community support.
• Function autonomously with the ability to take initiative and exercise good judgement and decision-making skills, particularly in a crisis.
• Demonstrate a calm and reassuring presence in emotionally-charged situations.
• Communicate effectively and frequently with supervisors; promptly alerting supervisor of concerns, seeking guidance/support when needed.
• Ability to work with multiple teams; such as, but not limited to school districts, county clinics and family members.
• Proficiency in using computer systems and databases for documentation and resource navigation.
• Maintains self in a professional manner at all times and adheres to the policies and procedures of the agency.
• Uphold confidentiality and adhere to ethical guidelines while handling sensitive information.
• Attends and actively participates in all required training, staff meetings and approved professional development opportunities to remain knowledgeable about best practices, policies, and protocols.
• Attends case conferences as requested by the school.
• Engages in supervision on a regular basis.
• Punctual and regular attendance is an essential responsibility of each employee.
• Employees are expected to report to work as scheduled, on time, and prepared to work
• Performs additional tasks as assigned.

Required qualifications

• Bachelor’s Degree in Social Work, Psychology, or other closely related human service field
• Minimum of two (2) years of experience working in a human services agency including one (1) year of supervised experience providing services in a mental health setting required. AND
• Maintain a valid New York State driver’s license and have access to reliable transportation
• Demonstrated knowledge of mental health and substance use issues affecting children and adults.
• Demonstrated knowledge of system-level issues facing clients, available resources, and the ability to effectively navigate these systems
