Northeast Kingdom Human Services (NKHS) in Vermont is seeking a Children's Case Manager to support youth and families, conducting assessments, developing treatment plans, and coordinating services across schools and community providers.

You will advocate for rights and needs, provide ongoing counseling and mentoring, monitor progress, and act as a liaison between Turning Points School, Cornerstone School, and Home and Community based programs.

Responsabilidades

• Conduct thorough assessments and collaborate with youth, families and professionals to develop a treatment plan.
• Serve as a central point of contact, coordinating services and resources for the child, youth and family.
• Advocate for rights and needs within the mental health system and help navigate complex systems.
• Monitor progress and provide ongoing counseling, mentoring, emotional support and encouragement.
• Provide immediate support during mental health emergencies as needed.
• Attend weekly meetings with Turning Points School and Cornerstone School to streamline services.
• Act as a liaison between schools and Home and Community based staff to ensure continuity of care.

This Case Management position will help to support children who attend Turning Points School and Cornerstone School in Orleans and Caledonia County and their families. Responsibilities

• Conducts thorough assessmentsand collaborates with the child/youth, their families, and other professionals to develop a comprehensive treatment plan.
• Serve as a central point of contact, coordinating various services and resources needed by the child/youth/family.
• Advocates for the child/youth/familys rights and needs with the mental health system. Helps child/youth/family navigate complex systems ensuring they have access to necessary services.
• Regularly monitor child/youth/family progress, ensuring they are adhering to their treatment plan and experiencing improvements to their mental health. Provides ongoing counseling, mentoring, emotional support, motivation, and encouragement to child/youth/family to help overcome barriers and challenges that may arise during treatment.
• Provides immediate support and intervention during mental health emergencies.
• Attend weekly meetings with Turning Points School and Cornerstone School to streamline services.
• Act as a liaison between schools and Home and Community based staff and programs to ensure continuity of care.
