Job summary

Sedgwick County is seeking a School Based Integrated Care Specialist II - COMCARE to provide goal‑directed, medically necessary case management for children with severe mental illness in a school setting. You will coordinate services, perform risk assessments, and support attendance at medical appointments and related activities.

The role requires a Bachelor's degree or equivalent by work experience, plus a valid US driver’s license and a vehicle for transport.

Qualifications

• Bachelor’s degree or equivalent by work experience.
• Must have a valid US driver’s license without restrictions and a personal vehicle for transportation.
• Meet CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.

Responsibilities

• Case management using identified methods to remediate symptoms and improve well‑being.
• Conduct risk assessments as needed to address safety of clients.
• Provide transportation in a personal vehicle to support attendance at medical appointments and related activities.
• Enter AIMS data as needed.
• Complete progress notes per COMCARE and CCBHC guidelines.
