Job summary

El Centro Family Health is seeking a Program Coordinator to sustain and expand SBHC School Base Health Center programs. You will oversee day-to-day operations, support the Health Programs Director, and collaborate to resolve issues while ensuring contract deliverables are met.

This role emphasizes data work, outreach, and clinical coordination across SBHC sites. The position requires a high school diploma or equivalent with 6 months to 1 year in healthcare or school environments, plus

Qualifications

• Six months to one year of experience in health care or school setting.
• Excellent verbal and written communication skills.
• Ability to pursue state-level CHW (CCHW) or CHES within 1–2 years.
• CPR or BLS certification within 90 days of hire.

Responsibilities

• Support SBHC program and Clinic Coordinators at SBHC sites and schools.
• Assist in scheduling to ensure full provider coverage.
• Handle data entry, reporting and analysis to inform operational plans.
• Support quality improvement and integrated care initiatives across sites.
• Attend and coordinate school/community events and outreach campaigns.
• Coordinate meetings with internal and external partners.
• Support patient care coordination with referral tracking and procedures.
• Complete trainings and adhere to PCMH and MU guidelines.
