Job summary

Together for Youth is seeking a Family Specialist to provide home- and school-based counseling and advocacy. You will assess families, coordinate treatment plans, and deliver case management to support youth well-being and permanency.

This role requires a Bachelor’s in human services, at least 1 year of relevant experience, a flexible schedule, and travel where needed. On-call shifts and collaboration with school staff are essential.

Qualifications

• Minimum Bachelor’s degree in human services or related field.
• At least 1 year of experience working with at-risk youth and families.
• Flexible schedule including day, evening, and on-call hours.
• Valid driver’s license with reliable vehicle; travel required.
• Bilingual is a plus!

Responsibilities

• Conducts assessment of family’s strengths and needs and utilizes resources to effect growth.
• Develops comprehensive treatment and discharge plans and evaluates progress.
• Delivers effective case management and schedules counseling sessions as needed.
• Tracks services and maintains timely, comprehensive documentation while upholding confidentiality.
• Conducts home and community visits at least twice a month to work toward goals.
• Develops relationships with schools and community agencies to support the child’s education.
• Engages youth and families to build trust and promote safety, stability and permanency.
