Job summary

Together for Youth is seeking a Prevention Family Specialist to preserve the family unit by providing supportive home-based casework counseling and empower parents with tools and resources to ensure children's safety, well-being and permanency.

The role conducts home assessments, develops treatment and discharge plans, and coordinates with schools and community agencies to support each child while maintaining confidentiality and thorough documentation.

Qualifications

• Minimum Bachelor’s degree in human services or related field.
• Minimum 1 year of experience working with at-risk youth and families and case management.
• Flexibility to work a flexible schedule with day, evening, and on-call hours; travel may be required.

Responsibilities

• Assess family’s strengths and needs; utilize treatment modalities and resources to effect growth and change.
• Develop, implement and evaluate comprehensive treatment and discharge plans; arrange permanency resources for youth.
• Provide case management; ensure counseling sessions; coordinate medical and dental exams; maintain thorough documentation.
• Conduct home and community visits at least twice a month; build strong relationships with youth, families, and school/community resources.
• Maintain relationships with schools and community resource agencies to ensure the best education for each child.
