Responsibilities

• Provide intensive outpatient case management in a school setting.
• Coordinate care with schools, families, and providers.
• Deliver direct, individualized interventions aligned with treatment plans.
• Maintain timely, accurate documentation and caseload management.
• Provide crisis intervention and follow-up, including missed appointments outreach.
• Conduct home visits as needed to engage youth and families.
