Adoration Health in Clarksville, TN is seeking a Home Health Scheduler to coordinate patient visits, assign clinicians, and maintain accurate schedules in a fast-paced, compassionate environment.

You will handle incoming calls, assist with referrals, and support the clinical team to ensure visits occur as ordered, while delivering outstanding service to patients and families. This role offers stability, teamwork, and opportunities for growth within a growing company.

Qualifications

• High school diploma or GED required.
• Minimum of two years of administrative experience, preferably in home health.
• Proficient in Microsoft Office (Word, Excel, Outlook) with ability to learn industry software.

Responsabilités

• Greet visitors in a courteous, professional manner.
• Answer incoming calls from patients, staff, and referral sources; transfer as needed.
• Communicate patient information to staff to ensure high-quality care.
• Select appropriate clinician for patient assignment based on location and skill.
• Provide administrative support, maintain supplies for staff and patient care.
• Assist clinical manager with managing patient schedules per frequencies and regulations.

Responsibilities

As a Home Health Scheduler, You will:

• Greet visitors in a courteous, professional, and timely manner
• Assist with answering incoming calls from patients, staff, physician offices, referral sources, etc., and transfer callers to the appropriate person or department
• Communicate patient information to appropriate staff as needed to provide the highest level of care
• Select an appropriate clinician for patient assignment based upon patient needs, location of the patient, and skill of the clinician
• Provide administrative support for the agency by performing general office duties, including maintaining an adequate stock of supplies needed for staff and patient care
• Support the clinical manager with managing patient schedules to ensure visits are performed in accordance with ordered frequencies and regulatory requirements

Qualifications

• High school graduate or GED
• Minimum of two-year of administrative experience, preferably in home health
• Proficient in the use of Microsoft Office (Word, Excel, Outlook, Internet Explorer) with the ability to learn industry specific software applications
• Ability to work independently and in a team environment
• Excellent, oral, written, and interpersonal communication skills
