Client Growth Resources, Inc. seeks a passionate Physical Therapist Assistant to provide in-home physical therapy in San Jose, CA. You will deliver targeted interventions, design personalized plans, and work closely with a multidisciplinary team to improve patient mobility and quality of life.

Must hold a CA PTA license, CA fingerprint clearance, and active BLS/ACLS, with up-to-date TB and vaccines. A valid driver’s license with auto-insurance is required for travel to patient homes.

Qualifications

• Licensed Physical Therapist Assistant in California.
• Current CA Fingerprint Clearance Card.
• BLS/ACLS certification current.
• Recent TB test and annual Flu shot.
• Immunization records including Covid-19, Hep B, MMR, TDaP and Varicella.
• Valid driver’s license with auto-insurance.

Responsabilités

• Deliver skilled PT interventions in patients' homes in San Jose, CA.
• Create and execute individualized treatment plans tailored to each patient.
• Collaborate with a multidisciplinary healthcare team for holistic care.
