Job summary

6 Degrees Search is seeking a passionate Physical Therapist Assistant to provide in-home physical therapy services in San Jose, CA. You will deliver skilled interventions, design personalized treatment plans, and collaborate with a multidisciplinary team to improve patients' mobility and quality of life.

The role requires a CA PTA license, CA fingerprint clearance, and current CPR/ACLS, TB testing, and up-to-date immunizations. A valid driver's license with auto-insurance is also required.

Qualifications

Current Physical Therapist Assistant license in the State of California.
Valid CA Fingerprint Clearance Card.
Up-to-date BLS (CPR) / ACLS certification.
Recent Negative TB test result and Flu Shot within the past year.
Comprehensive Immunization Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
Valid Driver's License along with auto-insurance.

Responsibilities

Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.
