Responsibilities

• Collects and analyzes data, identifies desired outcomes, and develops an individualized plan of care.
• Directs care and coordinates utilization of any nursing assistants.
• Collaborates with patient, family, physicians and staff; documents care plans.
• Participates in shared governance, committees, and unit projects.
• Evaluates patient responses to interventions and updates care plans accordingly.
• Utilizes evidence to guide nursing practice.
