Job summary

Far West Local Health District is seeking a Wellbeing and Health in-reach Nurse (WHIN) Coordinator CNS2 to support school-aged children and young people. The role provides expert clinical nursing in schools, coordinating health, wellbeing and social care services with multiple agencies.

The CNS2 will lead with autonomy, ensure high quality care, and foster interagency collaboration to improve outcomes for children and families in the Far West region.

Qualifications

• Current RN registration with 3+ years post-registration experience with children and young people.
• Post-registration qualification in child health/paediatrics, or adolescent health nursing.
• Demonstrated ability to engage families to address health problems and liaise with health services.

Responsibilities

• Coordinate identification of health, wellbeing and social needs for school-aged children.
• Provide complex health care coordination for students in designated schools.
• Lead interagency collaboration to access services across multiple agencies and professions.
