ONSIDE in Worcestershire seeks a compassionate Social Prescribing Link Worker to join our Adult Wellbeing Services. You will work with GP Practices within WF network to empower individuals to take control of their health by linking them to community-based support.

You will manage referrals, develop wellbeing plans, help people access local services, and collaborate with primary care teams while upholding GDPR and safeguarding.

Qualifications

• Level 3 qualification (A Level, BTEC Level 3 or equivalent).
• Experience supporting individuals within health, social care, community or public health settings.
• Experience with safeguarding and managing a caseload.

Responsabilités

• Receive and manage referrals from GP Practices and other sources.
• Provide personalised, one-to-one support using a holistic, person-centred approach.
• Develop wellbeing plans and co-produce achievable actions.
• Support access to local services and community groups.
• Maintain accurate records and monitor outcomes.
• Work with partner organisations to identify gaps in provision.
