Nflpediatrics seeks a Behavioral Health Care Coordinator to support rural patients in navigating mental health and substance use care, bridging gaps between behavioral health facilities and primary care providers.

You will develop personalized care plans, coordinate telehealth check-ins, connect clients to local clinics, and ensure documentation aligns with state and federal compliance. A Bachelor’s degree and 2–3 years of experience are preferred, with travel within rural regions.

Qualifikationen

• Bachelor's degree or higher in social work, psychology, counseling, public health, or related field
• 2–3+ years working with high-need populations, health navigation, or behavioral health services
• Proficiency with EHRs or care coordination software; strong communication abilities

Aufgaben

• Care Coordination: Develop personalized care plans, coordinate monthly telephonic/telehealth check-ins, and navigate patients through chronic care management (CCM).
• System Integration: Connect rural PCPs with regional psychiatric consultants, emergency services, and community support teams; guide virtual psychiatric visits.
• Cultural Competency: Maintain respectful, unbiased care for diverse rural clients and families.
• Documentation & Compliance: Maintain secure patient data and track wellness plans with clinicians in compliance.
• Crisis Follow-Up: Reach out after ER visits or crises to prevent readmission.
