Job summary

The Far West Local Health District is seeking a Palliative Care Registered Nurse to join the Specialist Community Palliative Care Team. You will deliver patient-centred palliative care for people with life-limiting illnesses and support families and carers.

The role involves a mix of direct, shared and consultative care across settings, collaborating with a multidisciplinary team to provide high-quality care in a rural environment.

Qualifications

• Current Authority to Practice as a Registered Nurse with AHPRA, with recent clinical experience relevant to palliative care.
• Demonstrated ability to practice per RN Standards of Practice, Code of Ethics, and professional conduct.
• Proven communication skills with diverse groups and capacity to work in a multidisciplinary team.

Responsibilities

• Provide patient-centred palliative care, direct/shared/consultative across settings.
• Coordinate with multidisciplinary team including primary/specialist care, community services and aged care.
