Qualifications

MSN or Doctoral degree required.
Current APRN license in the state of employment.
ELNEC training completed within 18 months of hire.
Certification in Hospice and Palliative Nursing preferred.
Valid driver’s license and reliable transportation.

Responsibilities

Perform comprehensive nursing assessments for palliative patients.
Communicate with physicians regarding patient updates and care plans.
Initiate and manage individualized care plans for patients and families.
Provide direct face-to-face visits for referred and existing patients.
Ensure compliance with state and federal regulations and JCAHO standards.
