Job summary

The CDC Foundation seeks a Rural Health Transformation CHW Support & Partnership Coordinator to advance statewide CHW programs in Kentucky. You will partner with pilot sites, public health agencies and community partners to promote evidence-based workflows, workforce development and sustainable CHW models across rural communities.

You will provide coaching, technical assistance and implementation support to CHWs, supervisors and pilot leadership, fostering collaboration while ensuring adherence

Qualifications

• 5+ years as a Community Health Worker, social worker, public health professional or related field.
• Bachelor's degree in public health or related field preferred; equivalent experience acceptable.
• Strong ability to communicate with diverse audiences and train others.
• Knowledge of CHW certification and renewal requirements in Kentucky.

Responsibilities

• Provide technical assistance to organizations implementing CHW initiatives and onboarding.
• Support integration of CHWs into clinical/public health settings with evidence-based practices.
• Deliver ongoing coaching and implementation support to CHWs and site leadership.
• Conduct site visits to assess progress and support quality improvement.
• Ensure CHWs operate within Kentucky scope of practice and program standards.
